Generic Name
Morphine sulfate 10mg, 20mg, 30mg, 50mg, 60mg, 80mg, 100mg, 200mg; sust-rel pellets in caps.
Indications
Prolonged relief of moderate-to-severe pain, when opioid analgesia is needed for more than a few days.
Children
<18 years: not recommended.
Adults
Swallow whole or may sprinkle contents on applesauce. See literature. Usually not for initial therapy; begin with an immediate-release product and convert to a sust-rel product. May be given on 24 hr or 12 hr schedule. Adjust dose at 2-day intervals.
Contraindications
Respiratory depression. Acute or severe asthma. Paralytic ileus.
Precautions
100mg and 200mg caps: for use in opioid-tolerant patients only. Head injury. Increased intracranial pressure. CNS depression. Coma. Toxic psychosis. Convulsive disorders. Shock. Impaired renal, hepatic, thyroid, pulmonary, or adrenocortical function. Prostatic hypertrophy. Urethral stricture. Acute abdomen. Biliary tract surgery. Cordotomy. Acute pancreatitis. Kyphoscoliosis. Drug abusers. Acute alcoholism. Delirium tremens. Elderly. Debilitated. Pregnancy (Cat.C). Labor & delivery, nursing mothers: not recommended.
Interactions
During or within 14 days of MAOIs: not recommended. Alcohol may compromise extended-release characteristics (contact manufacturer). Potentiation with alcohol, other CNS depressants, anticholinergics, tricyclic antidepressants, phenothiazines, general anesthetics, skeletal muscle relaxants. Antagonizes diuretics. Possible withdrawal symptoms with mixed agonist/antagonist opioids.
Adverse Reactions
Drowsiness, constipation, nausea, dizziness, anxiety, asthenia, GI disturbances, respiratory depression or arrest, circulatory depression, cardiac arrest, hypotension, shock.
