Generic Name
Morphine sulfate 30mg, 60mg, 90mg, 120mg; ext-rel caps (contains immediate-release + ext-rel beads).
Indications
Moderate to severe pain when continuous opioid analgesia is needed for an extended period of time. Not for as-needed use.
Children
<18 years: not recommended.
Adults
See literature. Swallow whole, or may sprinkle contents on applesauce (swallow immediately); do not crush, chew, or dissolve beads. Individualize. ≥18 years: opioid-naive: initially 30mg once daily; may increase by 30mg every 4 days. Switching from other oral morphine products: give total daily oral morphine dose as Avinza once daily. Switching from parenteral morphine or other opioids to Avinza, or switching from Avinza to other pain control methods: see literature. For all: max 1.6g/day (due to toxicity of excipients). Use 60mg and higher strengths for opioid-tolerant patients only.
Contraindications
Respiratory depression. Acute or severe bronchial asthma. Paralytic ileus. Avoid all alcohol (may be fatal).
Precautions
Not for post-op use. Head injury. Increased intracranial pressure. CNS depression. Shock. Volume depleted. Impaired renal, hepatic, thyroid, pulmonary, adrenocortical function. Pancreatic/biliary tract disease. Seizure disorders. GI or GU obstruction. Toxic psychosis. Delirium tremens. Acute alcoholism. Drug abusers. Avoid abrupt cessation. Reevaluate periodically. Elderly. Debilitated. Pregnancy (Cat.C). Labor & delivery, nursing mothers: not recommended.
Interactions
See Contraindications. Potentiation with alcohol, other CNS depressants, anticholinergics, tricyclic antidepressants, phenothiazines, general anesthetics, skeletal muscle relaxants, cimetidine. Do not take during or within 14 days of stopping MAOIs. Possible withdrawal symptoms with mixed agonist/antagonist opioids.
Adverse Reactions
Constipation, somnolence, GI upset, headache, edema, rash, sweating, infection, fever, dry mouth, insomnia, paresthesia, anorexia, depression, dyspnea, respiratory depression, orthostatic hypotension, syncope, euphoria.
