Pharmacological Class
Selective 5-HT1B/1D receptor agonist.
Generic Name
Naratriptan (as HCl) 1mg, 2.5mg; tabs.
Indications
Acute treatment of migraine.
Children
<18yrs: not recommended.
Adults
≥18yrs: 1mg or 2.5mg with fluids; may repeat once after 4 hours; max 5mg/24 hours. The safety of treating, on average, more than 4 headaches in a 30-day period has not been established. Mild-to-moderate renal or hepatic impairment: consider lower initial dose; max 2.5mg/24 hours.
Contraindications
Ischemic cardiac, cerebro- or peripheral vascular disease (eg, history of MI, angina pectoris, silent myocardial ischemia, stroke, TIA, ischemic bowel disease). Other significant cardiovascular disease. Uncontrolled hypertension. Severe renal or hepatic impairment. Basilar or hemiplegic migraine. Within 24 hours of other 5-HT1 agonists or ergot-type drugs.
Precautions
Confirm diagnosis. Exclude underlying cardiovascular disease, supervise 1st dose, and consider monitoring ECG in patients with likelihood of unrecognized coronary disease (eg, postmenopausal women, men over age 40, hypertension, hypercholesterolemia, obesity, diabetes, smokers, strong family history). Monitor cardiovascular function in long-term intermittent use. Peripheral vascular or colonic ischemia following other 5-HT1 agonists. Hepatic or renal dysfunction. Elderly: not recommended. Pregnancy (Cat.C). Nursing mothers.
Interactions
Methysergide, other ergotamines, or other 5-HT1 agonists: see Contraindications. Serotonin syndrome with SSRIs (eg, citalopram, escitalopram, fluoxetine, fluvoxamine, paroxetine, sertraline) or SNRIs (eg, duloxetine, venlafaxine).
Adverse Reactions
Paresthesias, dizziness, drowsiness, malaise/fatigue, throat/neck symptoms (pain, pressure, tightness), drug overuse headache (discontinue if occurs); rare: serious cardiac events.
