Pharmacological Class
Cephalosporin.
Generic Name
Cefotetan disodium 1g, 2g; IM or IV inj; sodium content 3.5mEq/g.
Indications
Susceptible lower respiratory tract or urinary tract, skin and skin structure, bone and joint, gynecologic, intraabdominal infections. Surgical prophylaxis.
Children
Not recommended.
Adults
Treatment: give for at least 5–10 days. UTI: 500mg IV or IM every 12 hours or 1–2g IV or IM every 12–24 hours. Mild to moderate skin and skin structures: 2g IV every 24 hours or 1g IV or IM every 12 hours. Severe skin and skin structure: 2g IV every 12 hours. Other sites: 1–2g IV or IM every 12 hours. Severe: 2g IV every 12 hours. Life threatening: 3g IV every 12 hours; max 6g/day. Prophylaxis: 1–2g IV once 30–60 minutes before surgery. CrCl <30mL/min: reduce dose, see literature.
Contraindications
Previous cephalosporin-associated hemolytic anemia.
Precautions
Penicillin or other allergy. Renal impairment. Renal or hepatic dysfunction, poor nutritional state, cancer patients, elderly or debilitated: monitor prothrombin time. Monitor for hemolytic anemia in prolonged use. GI disease (esp. colitis). Pregnancy (Cat.B). Nursing mothers.
Interactions
Avoid alcohol for 72 hours after dosing. Avoid other nephrotoxic drugs. Potentiated by probenecid. May cause false (+) Clinitest, Benedict's or Fehling's soln. May interfere with creatinine tests.
Adverse Reactions
GI upset, rash, pruritus, local reactions, anaphylaxis, blood dyscrasias, elevated liver enzymes.
