Generic Name
Trivalent, live attenuated influenza vaccine (virus types A and B); formulation changes annually; nasal spray; preservative-free.
Indications
Influenza immunization for healthy persons 2–49 years of age.
Adults and Children
<2yrs or ≥50yrs: not recommended. Give before start of flu season; each dose is 0.2mL intranasally (as 0.1mL/nostril). 2–8yrs (not previously vaccinated with flu vaccine): 2 doses/season, at least 1 month apart. 2–8yrs (previously vaccinated with flu vaccine) and 9–49yrs: 1 dose/season.
Contraindications
Allergy to egg, gentamicin, gelatin, or arginine. Life-threatening reaction to previous flu vaccine. Concomitant aspirin in patients 2–17yrs of age (Reye's syndrome).
Precautions
Have epinephrine inj (1:1000) available. Delay administration in acute febrile and/or respiratory illness. Asthma and children <5yrs old with recurrent wheezing: not recommended. Bronchospasm. Not for patients with chronic cardiovascular or pulmonary disorders, pregnant women who will be in 2nd or 3rd trimester during flu season, diabetes, renal dysfunction, other debilitating conditions; consider other products (eg, inactivated IM vaccine) for high-risk patients. History of Guillain-Barre syndrome. Immunodeficiency or risk of immunosuppression. Pregnancy (Cat.C). Nursing mothers.
Interactions
Do not administer until 48 hours after antiviral therapy cessation. Do not give antivirals within 2 weeks of administration.
Adverse Reactions
Transient flu-like symptoms (rhinorrhea, cough, irritability, headache, fever, sore throat, muscle aches, vomiting, fatigue), sinusitis.
