Platelet aggregation inhibitor.
Generic Name
Clopidogrel (as bisulfate) 75mg, 300mg; tabs.
Indications
Reduction of atherosclerotic events in: recent MI or stroke, peripheral arterial disease; non-ST-segment elevation acute coronary syndrome (unstable angina/non-Q-wave MI) or ST-segment elevation acute MI; see literature.
Children
Not recommended.
Adults
75mg once daily. Non-ST-segment acute coronary syndrome (give with aspirin 75–325mg once daily): give one 300mg loading dose first. ST-segment elevation acute MI (give with aspirin, with or without thrombolytics): may start with or without a loading dose.
Contraindications
Active pathologic bleeding (eg, peptic ulcer, intracranial hemorrhage).
Precautions
Risk of bleeding (eg, surgery, ulcers, trauma, concomitant NSAIDs). Severe hepatic or renal disease. Consider discontinuing 5 days before elective surgery. Pregnancy (Cat.B). Nursing mothers: not recommended.
Interactions
Caution with drugs that increase risk of bleeding (eg, NSAIDs, warfarin), and with drugs metabolized by CYP2C9 (eg, phenytoin, tolbutamide, tamoxifen, warfarin, torsemide, fluvastatin, many NSAIDs).
Adverse Reactions
Bleeding, GI upset/ulcers, bruising, rash, pruritus, dizziness, headache; rare: thrombotic thrombocytopenic purpura, neutropenia, agranulocytosis.
