Indications
Prevention of nausea & vomiting associated with emetogenic cancer chemotherapy, including initial and repeat courses. Prevention of post-op nausea & vomiting. Treatment of post-op nausea +/or vomiting (IV only).
Children
May give by IV inj as in adult dose, or may be diluted in apple or apple-grape juice and taken orally (timing and doses are different; see literature). <2 yrs: not recommended. 2–16 yrs: Chemotherapy-induced: 1.8mg/kg IV (max 100mg) about 30 minutes before chemotherapy; or, 1.8mg/kg (max 100mg) diluted in juice and taken orally within 1 hour before chemotherapy. Prevention: 0.35mg/kg IV (max 12.5mg) about 15 minutes before stopping anesthesia; or, 1.2mg/kg (max 100mg) diluted in apple or apple-grape juice and taken orally within 2 hrs before surgery. Treatment: 0.35mg/kg IV as soon as needed.
Adults
Give by IV inj, either undiluted 100mg over 30 seconds or diluted to 50mL and infused over up to 15 minutes. ≥16yrs: Chemotherapy-induced: 1.8mg/kg about 30 minutes before chemotherapy; or, may give 100mg IV. Prevention: 12.5mg about 15 minutes before stopping anesthesia. Treatment: 12.5mg as soon as needed.
Precautions
Patients at risk of developing prolongation of cardiac conduction intervals, especially QTc (e.g., congenital QT syndrome, hypokalemia, hypomagnesemia); see Interactions. Pregnancy (Cat.B). Nursing mothers.
Interactions
Caution with drugs which can prolong ECG intervals (esp. QTc) and with cumulative high-dose anthracycline therapy.
Adverse Reactions
Headache, dizziness, pain, fatigue, diarrhea, tachycardia, ECG changes.
