Generic Name
Hydrocortisone 100mg/60mL; enema.
Indications
Adjunct in ulcerative colitis.
Children
Not recommended.
Adults
1 enema nightly for 21 days or until symptoms controlled; discontinue if no improvement within 2–3 weeks.
Contraindications
Systemic fungal infections. Vaccinations. Postileocolostomy.
Precautions
Tuberculosis. Stress. Exacerbation or masking of infections. Diverticulitis. Impending perforation or abscess. Fresh intestinal anastomoses or obstruction. Fistulas. Sinus tracts. Peptic ulcer. Renal dysfunction. Hepatic cirrhosis. Hypertension. Osteoporosis. Myasthenia gravis. Hypo- or hyperthyroidism. Ocular herpes simplex. Psychosis. Monitor adrenal function and serum electrolytes. Avoid abrupt cessation after prolonged use. Pregnancy. Nursing mothers.
Interactions
May antagonize insulin or oral hypoglycemics, anticoagulants, aspirin. Hypokalemia with amphotericin B or diuretics.
Adverse Reactions
Hypertension, fluid retention, hypokalemia, adrenal suppression, poor wound healing, cataracts, glaucoma, psychosis, dysmenorrhea, myopathy, neuropathy, thrombophlebitis, peptic ulcer, convulsions, local irritation: folliculitis, hypertrichosis, acneiform eruptions, hypopigmentation, macerations, dermal and epidermal atrophy, secondary infections, striae, miliaria, local pain or burning.
