Indications
Central (cranial) diabetes insipidus. Temporary polyuria and polydipsia following head trauma or surgery in the pituitary region. Hemostasis in Hemophilia A, von Willebrand's disease (Type I), each with >5% Factor VIII.
Children
Diabetes insipidus: not recommended. Hemophilia A and von Willebrands: <3 months: not recommended. ≥3 months: as adult.
Adults
Diabetes insipidus: 2–4 micrograms/day SC or IV in 2 divided doses. If switching stabilized patient, use one-tenth of intranasal dose. Hemophilia A and von Willebrand's: 0.3 micrograms/kg IV over 15–30 minutes. If used preoperatively, give 30 minutes before scheduled procedure.
Contraindications
Moderate-to-severe renal impairment (CrCl<50mL/min).
Precautions
Monitor fluid intake, urine volume and osmolality. Fluid/electrolye imbalance (eg, cystic fibrosis). Coronary artery insufficiency. Hypertension. Nasal mucosal changes (nasal forms). Repeated administration of injectable before 48 hrs associated with tachyphylaxis. Very young. Elderly. Pregnancy (Cat.B). Nursing mothers.
Interactions
Caution with other pressor agents.
Adverse Reactions
Nasal congestion or rhinitis (nasal forms), headache, nausea, flushing, abdominal cramps, water intoxication, hyponatremia, changes in blood pressure; seizures in children from plasma hypoosmolality (rare).
