| Indications |
|
Forte: IM inj or intralesional use in steroid-responsive disorders when oral therapy not feasible. Intralesional: localized inflammation. |
| Adults and Children |
|
See literature. Infants: not recommended. Forte: usually 40mg IM/week. Intraarticular or intrasynovial: 5–40mg. Intralesional: usual max 12.5mg per site, 25mg per lesion, 75mg per week. |
| Contraindications |
|
Systemic mycoses. Live vaccination. |
| Precautions |
|
Avoid injection into previously infected or unstable joints. Tuberculosis. Latent amebiasis. Varicella. Vaccinia. Hypothyroidism. Cirrhosis. Ocular herpes simplex. If exposed to chickenpox or measles, consider prophylactic passive immune therapy. Renal insufficiency. Ulcerative colitis if perforation pending. Diverticulitis. Peptic ulcer. Hypertension. Osteoporosis. Diabetes. Supplement with additional steroids in physiologic stress. Avoid abrupt cessation. Alternate, intermittent or single-daily doses at 8AM minimize adrenal suppression. Monitor weight, growth, fluid and electrolyte balance. Pregnancy (Cat.C). Nursing mothers. |
| Interactions |
|
Barbiturates, hydantoins, rifampin may decrease effects. Avoid aspirin in hypoprothrombinemia. |
| Adverse Reactions |
|
HPA axis suppression, masks infection, increased susceptibility to infection, glaucoma, cataracts, secondary infections, hypokalemia, hypocalcemia, hypernatremia, hypertension, psychic disorders, myopathy, osteoporosis, peptic ulcer, dermal atrophy, increased intracranial pressure, carbohydrate intolerance. Parenteral: atrophy or flare (intraarticular use) at inj site. |
