Indications
Parkinsonism.
Children
Not recommended.
Adults
Take with food. Initially 1.25mg twice daily. May increase every 2–4 weeks by 2.5mg/day; max 100mg/day. Maintain levodopa dose at first.
Contraindications
Parkinsonism refractory to levodopa. Uncontrolled hypertension. Hypertension of pregnancy. Postpartum lactation. Other ergots.
Precautions
Dementia. MI. Arrhythmias. Renal or hepatic disease. Raynaud's syndrome. Discontinue if hypertension, severe headache, or CNS toxicity occurs. Psychosis. Peptic ulcer. GI bleed. Monitor hepatic, cardiovascular, renal function, BP, blood counts. Post-partum use if history of severe cardiovascular disease (eg, coronary artery disease). Pregnancy (Cat.B). Nursing mothers: not recommended.
Interactions
See Contraindications. Avoid phenothiazines, alcohol. Potentiates antihypertensives. Caution with dopamine antagonists, butyrophenones, drugs that affect BP. Antagonized by phenothiazines, haloperidol, metoclopramide, pimozide.
Adverse Reactions
GI upset, involuntary movements, hallucinations, confusion, on-off phenomenon, dizziness, drowsiness, insomnia, syncope, asthenia, visual disturbances, ataxia, hypotension; rarely: hypertension, seizure, stroke, MI.
