Indications
Hypertension.
Children
<18yrs: not recommended.
Adults
Not for initial therapy. May be substituted for titrated components. Individualize. ≥18yrs: BP not controlled on olmesartan or HCTZ alone: one tab once daily; may titrate at 2–4 week intervals; usual max 40mg/25mg once daily. Severe renal impairment (CrCl ≤ 30mL/min): not recommended. Volume depleted: reduce dose.
Contraindications
Anuria. Sulfonamide allergy. Pregnancy (Cat.D in 2nd and 3rd trimesters).
Precautions
Correct salt/volume depletion before starting, or monitor closely. Renal or hepatic dysfunction. Renal artery stenosis. Severe CHF. Asthma. Diabetes. Postsympathectomy. SLE. Gout. Monitor electrolytes. Elderly. Pregnancy (Cat.C in 1st trimester). Nursing mothers: not recommended.
Interactions
Digitalis, lithium toxicity. Adjust antidiabetic, antigout medications. Hyperkalemia with K+ supplements, K+ sparing diuretics, or K+ containing salt substitutes. ACTH, amphotericin B, corticosteroids increase hypokalemia risk. Orthostatic hypotension potentiated by alcohol, CNS depressants. Antagonized by NSAIDs. Potentiates other antihypertensives. May potentiate nondepolarizing muscle relaxants. May antagonize norepinephrine. May interfere with parathyroid tests.
Adverse Reactions
Dizziness, infection, hyperuricemia, nausea, back pain, weakness, orthostatic hypotension, electrolyte disturbances, rash, adverse lipid values, rhabdomyolysis (rare).
