Indications
Edema.
Children
Not recommended.
Adults
Give IV dose over a period of 2 minutes. Edema due to CHF: 10–20mg once daily oral or IV, if needed titrate upwards by doubling dose until desired response; max 200mg daily. Edema due to renal disease: 20mg once daily oral or IV, if needed titrate upwards by doubling dose until desired response; max 200mg daily. Edema due to hepatic disease: 5–10mg once daily oral or IV, give with aldosterone antagonist or potassium-sparing diuretic, may titrate upwards by doubling dose until desired response is achieved; max 40mg daily.
Contraindications
Anuria. Sulfonamide allergy.
Precautions
Hepatic disease with cirrhosis and ascites. Monitor electrolytes, BUN, creatinine, uric acid, and fluids. Pregnancy (Cat.B). Nursing mothers.
Interactions
Lithium and salicylate toxicity. Caution with NSAIDs. Antagonized by probenecid and indomethacin. Give oral dose 2 hours before or 4 hours after cholestyramine. Ototoxicity with aminoglycosides and ethacrynic acid.
Adverse Reactions
Dizziness, headache, nausea, weakness, vomiting, hyperglycemia, hyperuricemia, hypokalemia, excessive thirst, hypovolemia, impotence, esophageal hemorrhage, dyspepsia.
